Mission Statement:
School nurses are members of school educational teams striving for safety, good health and optimal learning of children and staff in their care. They act as an advocate for the students' emotional, physical and educational well-being.
Registration Health Packets
Below are the official required health forms.
School Immunization Requirements are based on NYSDOH amended regulations effective September 1, 2024:
DTaP (grades Pre- K-12)
- 5 doses of diphtheria and tetanus toxoid-containing vaccine and acellular pertussis vaccine (DTaP) K-12
- 4 doses if the 4th dose was received at 4 years of age or older K-12
- 3 doses if 7 years or older and the series was started at 1 year or older K-12
- 4 doses Pre - K
Polio (grades K -12)
- 3-4 doses, if the 3rd dose was received at 4 years or older K-12
- 3 doses Pre - K
Meningococcal (grades 7 - 12)
- 1 dose for grade 7,8, 9,10 and 11)
- 1-2 doses for grade 12 -1 dose if given on or after 16 years of age or 2 doses of vaccine with the booster dose given on or after 16 years of age
MMR (grades Pre-K-12)
- 2 doses K-12
- 1 dose Pre –K
Varicella (grades Pre- K -12)
- 2 doses K-12
- 1 dose Pre -K
Hepatitis B (grades Pre - K -12)
- 3 doses
Tdap (grades 6 -12)
- 1 dose
However only doses of Tdap given at age 10 years or older will satisfy the Tdap requirement for students in grades 6 through 10. Doses of Tdap given at age 7 years or older will satisfy the requirement for students in grades 11 and 12.
