Mission Statement:

School nurses are members of school educational teams striving for safety, good health and optimal learning of children and staff in their care. They act as an advocate for the students' emotional, physical and educational well-being.



IMPORTANT

School Immunization Requirements for the 2017-18 School Year based on NYSDOH amended regulations effective September 1, 2017:

MMR (grades K-12)

  • 2 doses of measles and mumps vaccines and 1 dose of rubella vaccine (MMR)

DTaP (grades K-12, except 10-12 as noted below*)

  • 5 doses of diphtheria and tetanus toxoid-containing vaccine and acellular pertussis vaccine (DTaP) required for entrance
  • If 4th dose received at 4 years of age or older, only 4 doses required
  • If 7 years or older and the series was started at 1 year or older, only 3 doses required

Polio (grades K, 1, 2,3,6,7, 8 & 9)

  • 3-4 doses as long as 1 dose is received after 4 years of age
  • If 3rd dose received at 4 years of age or older, only 3 doses required

Polio (grades 4 & 5, 10, 11 &12)

  • 3 doses

Meningococcal (grades 7, 8 & 12)

  • 1 dose for grade 7 & 8
  • 1-2 doses for grade 12 -1 dose if given on or after 16 years of age or 2 doses of vaccine with the booster dose given on or after 16 years of age

Varicella (grades K, 1, 2, 3,6,7,8 & 9)

  • 2 doses

Varicella (grades 4, 5, 10, 11 & 12)

  • 1 dose

Hepatitis B (grades K -12)

  • 3 doses

Tdap (grades 6 -12)

  • 1 dose 11 years of age or older

*Changes to Grades 10 through 12 School Entrance Immunization Requirements
Children enrolling in grades 10 through 12 in the 2017-18 school year are in this cohort, including new entrants in subsequent years into the cohort’s current grade levels (e.g. 2018-2019 grades 11-12, 2019-2020 12th grade)

  • Must meet immunization requirements of regulations in effect prior to July 1, 2014
  • Do not need to have intervals assessed for immunizations.
  • Are compliant for 2017-2018 through graduation if they meet requirements in effect June 30, 2014 which were:

DTaP/DTP 3 doses
Tdap 1 dose
Polio 3 doses
MMR 2 doses, or 2 doses of measles, 1 dose of mumps and 1 dose of rubella
Hep B 3 doses
