Instructions are included on each form. Please be sure to:

  1. Read deadline and filing instructions

  2. Print the forms (you must have Adobe Reader to view and/or print these pages ( http://www.adobe.com/products/acrobat/readstep2.html )

  3. Submit them to the School Nurse at your child's school.
