Kindergarten Registration

Your child may enter kindergarten in September if he/she will be five years old on or before December 31.

These are required to register your child:

1. A copy of your child's birth certificate or passport

2. Proof of residency: driver's license and car insurance card or electric and phone bills and, for renters, original lease and notarized landlord/property owner affidavit; for home owners, mortgage contract and/or tax bill

3. Proof of custody or guardianship, if applicable

4. Child's social security number

5. Completed registration packet for Health Office including:

v     Completed registration packet for the Health Office including:
o       Verification of immunizations (required by law) including:

• 3 doses of hepatitis B

• 3 doses of of DTaP or DT

• 3 doses of IPV

• 1 dose rubella

• 1 dose mumps

• 2 doses measles

•  1 dose varicella

o         Pink Health History form

o       Physical examination form to include BMI measurement

o       Tuberculin screening form

o       Dental certificate

o      Emergency card for Health Office

6. Alternate local emergency number (if you are not at home)

You may register your child between 8:30 a.m. and 3:30 p.m. while schools are in session.

Please fill out the Registration Packet either before or when you register.
